How (or from whom) did you learn of this Global Passion Ministries mission trip?
We do our best to make these trips as comfortable as possible. However, Global Passion mission trips may be extremely strenuous and stressful. They may include long airplane, train, boat, or bus rides of 10 to 20 hours in duration. Travelers are always required to carry their own luggage. Rest rooms are not always readily accessible. The food can be high in fat, carbohydrates, and sodium content. Fruits and vegetables may not be available. The housing and meeting rooms may not have air conditioning or adequate heating. There can be a considerable amount of walking between locations in addition to climbing many flights of stairs. During the winter months, walking may be on snow-covered or ice-covered walkways and stairs. Conditions in much of the world can be very hot and this might affect your overall strength and energy. The air quality is poor in many locations. Each trip varies. Please speak with the team leader if you have any concerns about your suitability for a particular trip. Any of these factors could aggravate certain health conditions and the medical facilities in many countries where we travel are limited. We reserve the right to request a medical release statement from your physician.
1. Do you have any existing medical condition(s) that may require extended medical treatment or surgery in the future? Have you had any knee or back problems? Yes No 2. Have you had any surgery or major health problems in the past 2 years? If so, please explain. 3. Have you ever experienced any psychological or emotional conditions? (ie: eating disorders, bipolar disorder, depression, etc.) No Yes (If yes, please explain and indicate if you are currently being treated by a physician) I would prefer a phone call to discuss this answer. If you did not list any conditions in questions 1-3, please skip to question 5. 4. In your opinion, would any of the conditions listed above limit your ability to perform the ministry for which you have applied? Yes No Have you discussed this trip with your physician? Yes No 5. Are you currently taking or do you regularly take any medications? If so, please list the drug name(s), what each drug treats, and which are prescription and non-prescription. No Yes 6. Do you have any special dietary needs? If so, please explain. No Yes 7. Do you snore or have any sleep disorders? (This is to help us determine your room assignment and will be kept confidential. If you are not able to have a roommate due to your snoring, you may be required to pay extra for a private room if needed.) No Yes 8. Please briefly summarize your health. Do you place any limits on yourself to avoid physical or medical problems? (Any hearing, vision, mobility limitations?) The purpose of GPM is the ministry of Jesus Christ and His church. Any available sightseeing and shopping will be permitted only if it coincides with the team's main purpose, but could be cancelled if not deemed convenient for travel, time, or if it hinders the ministry. Trip costs are based on fall 2013 prices and are subject to change. Teams may be switched in the event of any political, natural, or missions-related crisis. All donations received by GPM go toward project expenses. To receive a tax deduction, the IRS stipulates that the donor must release control of the money donated to the Non-profit organization. For this reason, money cannot be refunded and cannot be designated to a person. The individual will be a fundraiser and will get credit for raising funds equal to the price of the trip. If an individual is unable to participate in a GPM trip, the funds he/she has raised, less incurred expenses and administrative fees, will remain credited to his/her account for one year depending on date of cancellation. In the event of political unrest or natural disaster, GPM decides if and where to send a team. In the event that a team has cancelled for any reason, team members will be assigned by GPM to another trip of similar expense. GPM regulates conduct, dress, and Christian lifestyle. These are explained in the packet of information sent to accepted applicants. Team members, leaders, and staff adhere strictly to GPM policies and are subject to dismissal for disobedience without refund or reimbursement. Team members, leaders, and staff serve at their own risk and GPM is not liable in the event of sickness, accident, death, terrorist acts, or for transportation or any other expense involved with the events listed. The GPM experience includes intense physical activity including extended hiking, continuous walking, and strenuous choreography. We require all participants to be in good physical and mental condition and may request a physical exam and reference from a doctor (if needed).
